What is Home Health?

Our Speech Therapists help patients communicate and engage with friends, family and community, a vital aspect of overall health.

The Wilshire Home Health team has dedicated Speech Therapists who provide rehabilitation in improving cognitive difficulties, communication skills, problem solving and swallowing disorders.

Home Health Aids

It is tough to recover alone. Whether you are in recovery, or caregiving for a loved one, let us help you focus on your recovery by aiding in everyday tasks and care.

Wilshire Home Health aids work with your nurse or therapist to provide personal care and assist with bathing and grooming, personal hygiene, activities of daily living and strengthening.

Medication Management

You don’t have to do this alone.

The nurses at Wilshire Home Health can assist you with new and changed medication in conjunction with the rest of your treatment. We provide education on the medication ordered by your physician – the purpose of each one and when and how to take it.

Our team reconciles all your medications for every possible side effect and interaction to keep you safe and recovering.

Medical Social Workers

We care about every aspect of your health and recovery.

Medical social workers are an incredible part of our Wilshire Home Health team. They assist you and your family in resolving social and emotional obstacles, using the best community resources to meet your specific needs.

Registered Dietitians

Your nutritional health is key to your recovery process. Our dietitians can bring you the nourishment that best compliments your recovery process.

Our team provides individual guidance to our patients, families and caregivers, adapting it to the individual’s illness.

Skin and Wound Care

The best health care comes from a team of trained specialists working in collaboration.

Our Nursing Team has a Certified Wound and Skin Specialist who assesses the overall skin care needs of each patient. The Wound Care Specialist focuses on therapeutic interventions, both treatment and prevention.

Palliative Care

Palliative care is different than hospice care, because it is available to you at any time while you’re receiving treatment for your illness. The goal is to help you receive comfort care to improve your quality of life. This includes relief from symptoms like pain, medical side effects, shortness of breath, nausea, constipation, loss of appetite, fatigue, sleep and other symptoms. Click here to learn more.

Telehealth In-Home Monitoring

Let us help you communicate your health statues and health care needs with daily monitoring between you and your physician, nurse or therapist, right at home. Telehealth is complimentary to existing services and does not replace your in-home care and visits. The monitor is there to help you, your family and caregivers perform daily functions like blood pressure, weight, diabetic monitoring, skin care issues and medication management.
